What to expect at your first visit

At your first new patient appointment Dr. Kinneavy spends an hour with you performing a thorough examination to determine the nature of your problem. She will also provide a chiropractic adjustment as part of your treatment, if deemed appropriate. The doctor will explain to you what she finds and is happy to answer any questions you might have.

X-rays and other forms of imaging are not typically required, but if the doctor feels that further imaging is necessary you will be referred to a local imaging center. If you have current images (within 2-3 years) of the problem area, with any reports, please feel free to bring these with you to your first appointment.


You have the option of printing your New Patient paperwork and Additional Forms here; or you may plan to arrive at our office 15 – 20 minutes prior to your initial visit to fill out the necessary paperwork. If you print and fill out your paperwork prior to your appointment, please try to arrive 5 – 10 minutes early so that we can verify you have all of the correct forms.

Each patient is required to complete a New Patient Health History form. These forms are tailored for different types of patients. Please choose the form that is most appropriate for your situation:

Additional forms are also required and include: Informed Consent, Financial Policies, and the HIPAA Signature Form.

Please read through the HIPAA Privacy Notice 2016 then print and sign the additional patient forms.

We currently take United Heath Care Insurance and Humana. If you have one of these insurance providers, please use the following form to contact your insurance company to verify your chiropractic coverage and services.

Also, please remember to bring a valid Driver’s License and your Insurance Card so that we can make a copy for your file. 

These are Pain Index forms that the doctor may also require that you print and fill out for your initial visit. As you schedule your appointment with us, we will let you know if either of these 2 forms should be part of your paperwork.
