What to expect on your first visit
Dr. Kinneavy will spend about an hour with you and during this time the she will perform a thorough examination to determine the nature of your problem and will typically take the time to provide treatment at this appointment. The doctor will explain to you what she finds and answer any questions you may have.
X-rays and other forms of imaging are not routinely done, however, if further imaging is indicated the doctor will refer you out to the appropriate place.
NEW PATIENT PAPERWORK
We ask that all new patients arrive at our office 15 – 20 minutes prior to your appointment to fill out necessary paperwork, or 5 – 10 minutes prior to appointment if you print and fill out your forms before your scheduled appointment. (Please note that if you forget your paperwork we ask that you arrive early)
Each patient is required to complete a New Patient History form. These forms are tailored for different types of patients. Please choose the form that is most appropriate for your situation:
- New Patient Child/Adult (age 6 and over)
- New Patient Baby/Child (under age 6)
- New Patient Auto Accident/Personal Injury
Additional forms include: Informed Consent, Financial Policies, and the HIPAA Signature Form. Please read through the HIPAA Privacy Notice 2016 then print and sign the additional patient forms.
We currently take United Heath Care Insurance and Humana. If your have one of these insurance providers, please use the following form to contact your insurance company.
Also, please remember to bring a valid Driver’s License and your Insurance Card so that we can make a copy for your file.
These are Pain Index forms that the doctor may also require that you print and fill out for your initial visit. As you schedule your appointment with us, we will let you know if either of these 2 forms should be part of your paperwork.
