What to expect on your first visit
We ask that you arrive at the office at your scheduled appointment time. This will allow for 15 minutes for you to fill out the needed paper work (or click on one of the links below to print out your paperwork and fill it out at home prior to your visit). Allow for about an hour of time to be spent with the doctor. During this time the doctor will perform a thorough examination to determine the nature of your problem and will normally provide treatment on the same day. The doctor will always take the time to explain to you what he or she finds and to answer any questions you may have.
X-rays and other forms of imaging are not routinely done, however, if further imaging is indicated the doctor will refer you out to the appropriate place.
NEW PATIENT PAPERWORK
Each person is required to complete a New Patient History form. The form is different for different types of patients. Please choose the form that's appropriate for your situation:
- New Patient (age 6 and over)
- Child/Baby New Patient (under age 6)
- Auto Accident / Personal Injury New Patient
Additionally, there are several other forms that all patients need to fill out:
- Basic Forms Packet -- required for all patients. This packet includes all of the individual forms below which can be downloaded individually as needed.

- Financial Policy
- Patient Insurance Verification
